Thomas Blissenbach, RPh, MS

Thomas Blissenbach, RPh, MS Mr. Blissenbach is currently Director of Business Development for Fairview Pharmacy Services, LLC. He has developed and led numerous ambulatory pharmacy business units at FPS since 2000. Mr. Blissenbach has more than 40 years experience in both hospital and alternate site pharmacy. He was a pioneer in the growth of Home Infusion and the evolution of Specialty Pharmacy. His work history includes positions involving multiple-site pharmacy management, sales and marketing, business development and consulting. Throughout his career he has consistently been a leader and innovator in the practice and business of pharmacy. Previous employers include HMSS, Caremark and Chronimed.


Mike Ellis, BSPharm, RPh
Corporate Vice President, Specialty Pharmacy & Infusion
Mike Ellis is Corporate Vice President, Specialty Pharmacy and Infusion for Walgreens. Before joining Walgreens in October of 2011, he served as the Senior Vice President of Specialty Trade Relations and Contracting and Pharmaceutical Services for CVS Caremark. His more than 29 years of experience covers both the Specialty Pharmacy and Infusion services industries, including senior positions with companies including AdvancePCS, Olsten Health Services, Caremark, CVS, and Quantum Health Resources. Ellis is a licensed pharmacist and earned his Bachelor of Science in pharmacy from the University of Arkansas, College of Pharmacy.

Walter S. Geba, MS, PharmD
Senior Vice President, Contracting Services & Chief Pharmacy Officer
Innovatix, LLC, and Essansa

Dr. Geba is responsible for providing leadership to the pharmaceutical contracting, medical/surgical contracting, clinical pharmacy services, and Innovatix Network, LLC teams and for the development and maintenance of contracting strategies, as well as expanding relationships with manufacturers, distributors, wholesalers, and prescription drug plans. Geba brings a wealth of knowledge and experience in pharmaceutical management, health care operations, and supply chain. Prior to joining Innovatix, Dr. Geba served as Corporate Director, Pharmacy Business Development with Continuum Health Partners – one of the largest integrated health care delivery systems in the New York metropolitan area. He also led Pharmacy Operations at Beth Israel Medical Center and Montefiore Medical Center in New York City. Dr. Geba holds a BS in Pharmacy from St. John’s University, a MS in Pharmacy Administration from Long Island University, and a Doctorate in Pharmacy from the University of Florida.

Phil Hagerman, RPh

He holds an honorary doctorate from Baker College.

Erik Halstrom, MHA

A pharmaceutical industry veteran, Erik has over 23 years of healthcare experience working at large pharmaceutical/biotech companies and smaller healthcare companies focused on the strategic needs of pharma/biotech. Prior to joining MHA as the Vice President of Specialty Pharmacy Solutions, Erik was Vice President of Business Development and a member of the management team at FFF Enterprises. Prior to FFF, Erik held a broad range of positions of increasing responsibility at Eli Lilly, and subsequently Merck. His past experiences include: sales, product marketing, pharmaceutical and vaccine customer marketing, new product strategy development, institutional segment marketing, managed care contracting, specialty pharmacy and strategic distribution.

Dr. Scott Howell
Senior Vice President of Clinical Affairs
Cardinal Health Specialty Solutions

Dr. Scott Howell has recently joined Cardinal Health Specialty Solutions as Senior Vice President of Clinical Affairs, responsible for overseeing the group’s clinical strategies, decisions, and operations.

Dr. Howell comes to CHSS from Genentech, a leading biotechnology company, where over the past seven years he held a variety of positions in contracting, distribution, customer operations, reimbursement support and patient assistance, marketing, and clinical development. Previously, Dr. Howell was Vice President of Pharmacy Affairs at Highmark BCBS where he led the pharmacy program, including the launch of its Part D program and its efforts in specialty pharmacy management and oncology. Prior to that, Dr. Howell worked at SmithKline Beecham/GSK in managed care medical affairs and a variety of sales and account management leadership positions.

Dr. Howell began his career as a general internist and worked several years in private practice and then several years in academic medicine at Ohio State University. At Ohio State he held a variety of leadership positions in group practice management and health plan administration, in addition to his clinical work and teaching.

Dr. Howell earned his undergraduate degree at Notre Dame and his medical degree at Ohio State. He completed his internship and residency and obtained a masters in business administration at Duke University. Dr. Howell is a frequent speaker at industry meetings and has been lecturing for several years at the graduate programs at the University of California, Berkeley.

James R. Lang, PharmD, MBA

Dr. Lang has 30 years experience in managing pharmacy programs, serving as a senior executive in a variety of health care settings that include pharmacy benefit management companies, managed care, and hospital systems. Currently Dr. Lang is the Vice President of Pharmacy Services for Blue Cross Blue Shield of Michigan, a health plan that serves approximately 4.5 million lives and has a drug spend of $5 billion. Most recently James served as President of Anthem Prescription Management where he grew the company from serving two million members to over seven million members. Prior to joining Anthem, he was the President of Healthsource Rx, a subsidiary of Healthsource that provided pharmacy benefit management services for approximately 1.5 million members in fifteen states.

W. Craig Miller

Craig Miller is President of US Bioservices, an AmerisourceBergen company where he leads the company’s specialty pharmacy division creating access and solutions for manufacturers, payers, providers and patients in the delivery of complex specialty biotech regimens. AmerisourceBergen is one of the world's largest pharmaceutical service companies serving the United States, Canada and selected global markets with a focus on the pharmaceutical supply channel. He has been with the organization for 7 years and previously served the role of Vice President, Sales and Business Development. He is responsible for all operations, commercial activities, innovative solutions and product development for US Bioservices. He also actively serves as a member of the AmerisourceBergen Specialty Group Executive Team. Craig brings over 23 years of experience in healthcare serving in senior leadership positions with both manufacturers and specialty pharmacy providers with primary focus on commercialization of products for rare disorders. Craig’s background ranges from biotech startups, including Jazz Pharmaceuticals and Viropharma Inc., to the specialty pharmacy division of Fortune 18 Company, CVS Caremark. He serves on industry advisory boards and takes an active role in local and national charities dedicated to improving patient care.

Mark Montgomery
President & Chief Executive Officer

President and CEO of Axium Healthcare Pharmacy, Mark Montgomery is a 25-year innovator in the specialty pharmacy sector of the healthcare industry. He joined Axium in 2003 as Vice President/General Manager, overseeing the daily operations for Axium. He was promoted to President in 2005. Prior to joining Axium, Mark played instrumental roles while employed with leading national specialty pharmacy providers, Accredo Health and Caremark. Mark currently serves on the Board of Directors of the Avant Healthcare and Florida Hospital Cardiovascular Institute, and is accredited by the Accreditation Commission for Health Care (ACHC). He is also a member of the Executive Committee for the 2013 March of Dimes, and actively participates in many Central Florida organizations. In 2012, Mark was recognized as one of the "Top 30 Most Influential People in Specialty Pharmacy” by Specialty Pharmacy Times.

Michael A. Nameth, RPh, MBA

Mr. Nameth is an accomplished senior level healthcare business leader with over twenty-five years of experience in several key areas of healthcare including: Managed Care (Health Plans), PBM (Pharmacy Benefit Management) and Specialty Pharmacy industries. Most recently, Mr. Nameth was named CEO of Town Total Health, a community based specialty pharmacy in New York. Prior to this appointment, he was the executive vice president for Walgreens Specialty Pharmacy. He also served as executive vice president at Medmark Specialty Pharmacy, a Walgreens acquisition in 2006. Prior to joining Medmark, he spent 10 years at WellPoint, the nation’s largest health benefits company and was president of its PBM. WellPoint’s PBM was the largest health plan owned PBM in the country. Prior to WellPoint, he headed up the pharmacy program at BCBS of Michigan and also started the Anthem BCBS PBM and built the first BCBS owned mail order pharmacy in Cincinnati, Ohio. He is a pharmacy school graduate of The Ohio State University and holds a Masters in Business Administration degree from the University of Dayton in Dayton, Ohio.
Diane E. Sullivan

Diane Sullivan joined AstraZeneca on September 3, 2013 as Vice President of Managed Markets, where she is responsible for leading the Managed Markets team. She brings extensive managed care experience in both Primary and Specialty Care as well as diverse leadership in both the pharmaceutical industry and technology organizations.

Prior to AstraZeneca, Sullivan served as as Vice President, US Specialty Care Payer and Channel Group at Pfizer. For her four years at Pfizer, Sullivan led Pfizer's US Specialty Care Strategy for payers, managed care organizations and specialty pharmacy across six brand franchises. Before that, she was Vice President of Marketing, Health Systems, at Wyeth, where she led an integrated strategic plan by focusing on growth strategies that targeted the top 20 customers that accounted for 80 percent of US lives and Wyeth sales of $10 billion. She also spearheaded efforts to improve payer marketing capabilities that led to the approval of a Payer Center of Excellence. She is on the Advisory Boards of the Professional Women’s Roundtable and the United Way Women’s Initiative. In addition, she previously served on the Boards of Leadership Philadelphia and the Support Center for Child Advocates, and remains involved in both Philadelphia-based non-profits.

Burt Zweigenhaft
