Thomas Blissenbach, RPh, MS Mr. Blissenbach is currently Director of Business Development for Fairview Pharmacy Services, LLC. He has developed and led numerous ambulatory pharmacy business units at FPS since 2000. Mr. Blissenbach has more than 40 years experience in both hospital and alternate site pharmacy. He was a pioneer in the growth of Home Infusion and the evolution of Specialty Pharmacy. His work history includes positions involving multiple-site pharmacy management, sales and marketing, business development and consulting. Throughout his career he has consistently been a leader and innovator in the practice and business of pharmacy. Previous employers include HMSS, Caremark and Chronimed.

Mike Ellis is Corporate Vice President, Specialty Pharmacy and Infusion for Walgreens. Before joining Walgreens in October of 2011, he served as the Senior Vice President of Specialty Trade Relations and Contracting and Pharmaceutical Services for CVS Caremark. His more than 29 years of experience covers both the Specialty Pharmacy and Infusion services industries, including senior positions with companies including AdvancePCS, Olsten Health Services, Caremark, CVS, and Quantum Health Resources. Ellis is a licensed pharmacist and earned his Bachelor of Science in pharmacy from the University of Arkansas, College of Pharmacy. |  | Walter S. Geba, MS, PharmD Senior Vice President, Contracting Services & Chief Pharmacy Officer Innovatix, LLC, and Essansa Dr. Geba is responsible for providing leadership to the pharmaceutical contracting, medical/surgical contracting, clinical pharmacy services, and Innovatix Network, LLC teams and for the development and maintenance of contracting strategies, as well as expanding relationships with manufacturers, distributors, wholesalers, and prescription drug plans. Geba brings a wealth of knowledge and experience in pharmaceutical management, health care operations, and supply chain. Prior to joining Innovatix, Dr. Geba served as Corporate Director, Pharmacy Business Development with Continuum Health Partners – one of the largest integrated health care delivery systems in the New York metropolitan area. He also led Pharmacy Operations at Beth Israel Medical Center and Montefiore Medical Center in New York City. Dr. Geba holds a BS in Pharmacy from St. John’s University, a MS in Pharmacy Administration from Long Island University, and a Doctorate in Pharmacy from the University of Florida.

Prior to joining Anthem, he was the President of Healthsource Rx, a subsidiary of Healthsource that provided pharmacy benefit management services for approximately 1.5 million members in fifteen states.

He has been with the organization for 7 years and previously served the role of Vice President, Sales and Business Development. President and CEO of Axium Healthcare Pharmacy, Mark Montgomery is a 25-year innovator in the specialty pharmacy sector of the healthcare industry. He joined Axium in 2003 as Vice President/General Manager, overseeing the daily operations for Axium. He was promoted to President in 2005. Prior to joining Axium, Mark played instrumental roles while employed with leading national specialty pharmacy providers, Accredo Health and Caremark. Mr. Nameth is an accomplished senior level healthcare business leader with over twenty-five years of experience in several key areas of healthcare including: Managed Care (Health Plans), PBM (Pharmacy Benefit Management) and Specialty Pharmacy industries. Most recently, Mr. Nameth was named CEO of Town Total Health, a community based specialty pharmacy in New York. Prior to this appointment, he was the executive vice president for Walgreens Specialty Pharmacy. He also served as executive vice president at Medmark Specialty Pharmacy, a Walgreens acquisition in 2006. Prior to joining Medmark, he spent 10 years at WellPoint, the nation’s largest health benefits company and was president of its PBM. WellPoint’s PBM was the largest health plan owned PBM in the country. Prior to WellPoint, he headed up the pharmacy program at BCBS of Michigan and also started the Anthem BCBS PBM and built the first BCBS owned mail order pharmacy in Cincinnati, Ohio.
